A massive fire that tore through a 24-story apartment building in west London has claimed a number of lives, fire officials confirmed, as rescuers frantically worked to reach people believed still trapped.

It's also surfaced on social media that fire safety advice inside the building told residents to stay in their apartments if there was ever a fire.

London mayor Sadiq Khan said the fire was a "serious incident" and suggested there are "questions that have to be answered", after several residents reported not hearing any alarms.

Edward Daffarn, who lived on the 16th floor, said the building's fire alarm didn't ring.

"We used to say that... it'll take a tragedy before the people wake up and before the people managing the building get held to account", Collins told AFP, as the 24-storey building smouldered nearby.

Another witness, Hanan, who gave only her first name, told ABC News that she managed to escape from the building's ninth floor and that she was anxious about her brother, Abdul Aziz, and his family, who were on the 26th floor.

It was determined the Lacrosse fire was started by a cigarette on a cluttered balcony, according a report into the blaze by the Metropolitan Fire Brigade.

It is managed by the Kensington and Chelsea Tenant Management Organization, which spent 10 million pounds ($12.8 million) refurbishing the building over the last two years.

"Firefighters wearing breathing apparatus are working extremely hard in very difficult conditions to tackle this fire", he said in a post on the brigade's Facebook page.

"And just outpouring of the most extraordinary love and generosity, and people just getting stuck in - putting their own interests aside".

London Fire Commissioner Dany Cotton earlier told reporters: "This is an unprecedented incident".
